What Is Guided Biofilm Therapy?

Dental biofilm — commonly called plaque — is a near-invisible bacterial film that forms continuously on teeth, gum tissue and implant surfaces. Left undisturbed, it hardens into calculus (tartar) and drives gum disease, tooth decay and peri-implant complications. Traditional cleaning instruments scrape and scale to remove it. GBT does this differently: it uses a structured sequence of precision technologies to identify, visualise and remove biofilm systematically, with less trauma to surrounding tissue.

GBT is a clinically validated protocol developed by EMS (Electro Medical Systems), using AIRFLOW® and PIEZON® technologies. Smile Specialists Suite applies this protocol as part of periodontal treatment for both prevention and active gum disease management.

How GBT Works: The Step-by-Step Process

GBT follows a consistent, evidence-based sequence adapted to each patient’s clinical needs:

  1. Examine — the periodontist assesses gum pocket depths, bleeding on probing, plaque levels and individual risk factors to determine the right treatment protocol.
  2. Disclose — a harmless vegetable-based dye is applied to the teeth and gums. It stains biofilm a bright colour, making it clearly visible. Patients can see exactly where plaque has accumulated — including areas that are easy to miss during home brushing.
  3. Motivate — the clinician uses the disclosed biofilm as a teaching tool, demonstrating where deposits are concentrated and how to improve home care technique for those specific areas.
  4. AIRFLOW® — a controlled jet of warm water, air and fine erythritol powder removes biofilm and staining from tooth surfaces, gum margins and below the gumline without abrasion. For patients with deeper periodontal pockets, PERIOFLOW® delivers this treatment precisely into the pocket without damaging the surrounding tissue.
  5. PIEZON® — ultrasonic instruments remove any remaining calculus with minimal contact pressure, using the PS No Pain setting to maximise comfort. A final clinical check confirms all biofilm and deposits have been cleared.

Who Benefits from GBT?

GBT is appropriate for most patients seeking professional cleaning, but is particularly effective for:

Patients with active gum disease or periodontitis requiring periodontal debridement as part of a treatment programme. Patients with dental implants — the AIRFLOW® system removes biofilm from implant surfaces without scratching them, which conventional metal instruments can do. Patients who experience discomfort or anxiety during routine cleaning. Patients on periodontal maintenance who need thorough, regular professional care. Patients with orthodontic appliances, crowns, bridges or veneers, where traditional instruments may be difficult to use safely.

Patients with peri-implant disease in particular benefit from GBT because biofilm on titanium implant surfaces must be removed without damaging the surface finish — a requirement GBT meets more effectively than conventional metal scalers.

Frequently Asked Questions

Guided Biofilm Therapy (GBT) is a systematic, technology-driven protocol for removing dental biofilm (plaque) and calculus. It uses AIRFLOW® and PIEZON® technologies to clean teeth more thoroughly and comfortably than traditional scaling and polishing.

GBT is more precise and generally more comfortable than a conventional scale and cleaner. The disclosure step makes biofilm visible before removal, which leads to more thorough cleaning. The AIRFLOW® system is gentler on gum tissue and tooth surfaces than metal scalers and reaches areas that traditional instruments cannot access easily.

Yes. GBT is particularly well-suited to patients with implants because the AIRFLOW® technology removes biofilm from implant surfaces without scratching or damaging the titanium. Conventional metal instruments can mark implant surfaces, which may increase the risk of bacterial re-attachment.

Most patients find GBT significantly more comfortable than traditional cleaning. The AIRFLOW® treatment is gentle and uses warm water. PIEZON® instruments apply minimal pressure. Patients who normally experience sensitivity during cleaning often report a noticeably more comfortable experience with GBT.

A GBT session typically takes between 45 and 90 minutes, depending on the extent of biofilm, the number of teeth, and whether any periodontal pockets require deeper treatment. The additional time spent on disclosure and motivation means the overall cleaning is more thorough and education-focused.

The recommended recall interval depends on your individual periodontal risk. Most patients on maintenance benefit from a session every three to six months. Patients with active gum disease may require more frequent appointments during the initial treatment phase. Your periodontist will advise the appropriate schedule based on your clinical assessment.
